Nutrition, Food Security, and Public Health in Africa

Nutrition — the intake of food and nutrients necessary for health, growth, and functioning — is foundational to every dimension of human development and public health. Across Africa, nutrition challenges take multiple interrelated forms: acute and chronic undernutrition affecting children and pregnant women, micronutrient deficiencies that impair development and immune function even in populations with adequate calorie intake, and a growing burden of overweight, obesity, and diet-related non-communicable disease associated with urbanisation and dietary transition. These overlapping challenges — sometimes described as the triple burden of malnutrition — coexist in the same countries and sometimes in the same households, requiring public health responses that go beyond simple calorie provision to address the quality, diversity, and safety of diets.

Undernutrition and Its Development Consequences

Undernutrition encompasses several distinct conditions with different causes and consequences. Stunting — low height for age reflecting chronic nutritional deprivation, most commonly from conception through the first two years of life — affects a substantial proportion of children under five in many African countries, with consequences that include impaired cognitive development, reduced educational attainment, lower adult productivity, and increased susceptibility to chronic disease in later life. These consequences are largely irreversible once the critical early childhood window has passed, making prevention through adequate maternal and infant nutrition during the first 1,000 days from conception a particularly high-priority investment. Wasting — low weight for height reflecting acute malnutrition — is a medical emergency requiring immediate therapeutic intervention and carries a high risk of death when severe. Micronutrient deficiencies — insufficient intake of specific vitamins and minerals including vitamin A, iron, iodine, and zinc — affect large numbers of people across the continent, impairing immune function, causing anaemia, impeding physical and cognitive development, and increasing susceptibility to infection even without visible signs of undernutrition.

Food Security: Availability, Access, and Utilisation

Food security — reliable access by all people at all times to sufficient, safe, and nutritious food — is shaped by the intersection of food availability, economic access, and biological utilisation. Food availability reflects agricultural production and food system performance: whether enough food is produced or can be imported to meet population needs. Economic access reflects whether households have sufficient income or assets to purchase or produce the food they need, and whether food markets function efficiently enough to distribute food from where it is produced to where it is needed at accessible prices. Biological utilisation reflects whether the food consumed can be effectively absorbed and used by the body, which depends on overall health status — infections and intestinal parasites impair nutrient absorption, and repeated infections in early childhood are a major driver of stunting even in households with adequate food access. Addressing food insecurity comprehensively requires attention to all three dimensions rather than focusing exclusively on agricultural production.

Agricultural Productivity and Food Systems

For a continent where a large proportion of the food consumed is produced by smallholder farmers, agricultural productivity is directly linked to household nutrition outcomes. Low yields, post-harvest losses, limited dietary diversity in production, and inadequate access to markets where diverse foods are available all constrain the nutritional quality of rural diets. Agricultural interventions that increase production of nutritionally valuable crops — legumes, vegetables, and fruits — alongside calorie-providing staples, and that reduce post-harvest losses that diminish the nutritional value of harvested food, contribute to improved household nutrition. Biofortification — developing and promoting crop varieties with enhanced micronutrient content — is a specific strategy that has demonstrated nutritional impact in several African contexts by improving the micronutrient density of staple crops that are already widely consumed.

Nutrition in the First 1,000 Days

The period from conception to a child’s second birthday is recognised as the most critical window for nutrition investment, with interventions during this period having the greatest and most lasting impact on child development and long-term health. Evidence-based interventions targeting this window include ensuring adequate maternal nutrition during pregnancy, promoting exclusive breastfeeding for the first six months of life, supporting appropriate complementary feeding with nutrient-rich foods from six months, and providing micronutrient supplementation for both mothers and young children. These interventions are delivered through antenatal care, postnatal services, and community health worker programs, and their reach and quality directly determine what proportion of children receive adequate nutritional support during this critical developmental window.

Dietary Transition and the Double Burden

Urbanisation and rising incomes across African countries are driving rapid changes in dietary patterns that present new nutrition challenges alongside persisting undernutrition. Traditional diverse diets based on locally produced whole foods are being replaced in many urban contexts by diets higher in processed foods, refined carbohydrates, added sugars, and unhealthy fats — dietary patterns strongly associated with rising rates of overweight, obesity, type 2 diabetes, hypertension, and cardiovascular disease. The speed of dietary transition varies across income levels, geography, and generations, but the trajectory is broadly consistent: as incomes rise and urban food environments change, diet-related non-communicable disease risk grows. Addressing this double burden — simultaneously addressing remaining undernutrition while preventing the diet-related chronic disease growth that nutritional transition brings — requires nutrition policy approaches that go beyond child undernutrition to encompass the food environment, food labelling, taxation of unhealthy products, and urban food system development.

Food Safety

Food safety — the conditions and practices that prevent food contamination and foodborne illness — is an important and somewhat underappreciated dimension of nutrition and public health across Africa. Foodborne illnesses from microbial contamination of food — bacteria, viruses, parasites, and naturally occurring toxins — cause very large numbers of preventable illnesses and deaths annually across the continent, disproportionately affecting young children. Aflatoxin contamination of grains and groundnuts — produced by moulds in improperly stored crops — is a particular concern across several African agricultural zones, causing both acute illness at high exposure and increased cancer and immune impairment risk at chronic lower levels of exposure. Strengthening food safety along the entire food value chain — from farm storage and handling through processing, transport, retail, and food service preparation — requires regulatory frameworks, enforcement capacity, and the practical infrastructure of cold storage, food testing laboratories, and food handler training.

Looking Ahead

Progress on nutrition across Africa will require coordinated action across agriculture, health, social protection, education, water and sanitation, and food systems — recognising that nutrition outcomes are determined by factors well beyond the health sector alone. Investment in the first 1,000 days remains the highest-return nutritional investment given its irreversible developmental consequences. Simultaneously addressing the rising burden of diet-related non-communicable disease will require earlier and more systematic policy attention to urban food environments and dietary transition than has yet been applied across most of the continent. And ensuring that agricultural transformation improves the nutritional quality of diets, not only calorie availability, is an increasingly important dimension of food system development strategy.
