Water and Health in Africa: Why Clean Water Is Your Most Important Medicine

Water is life. The human body is approximately 60% water. Every cell, tissue, and organ depends on it. Yet across sub-Saharan Africa, hundreds of millions lack reliable access to safe drinking water — a fundamental driver of preventable illness and death.

What Makes Water Unsafe?

  • Biological — Bacteria (E. coli, Vibrio cholerae), viruses (hepatitis A, rotavirus), protozoa (Giardia), and worm eggs from faecal contamination
  • Chemical — Fluoride, arsenic, nitrates, pesticides, heavy metals from agricultural runoff or geology
  • Physical — Turbidity from suspended particles that harbour microorganisms

Key Water-Related Diseases in Africa

Diarrhoeal disease is the second leading cause of death in children under five. Oral rehydration therapy (ORT) — water, salt, and sugar — is the life-saving first-line treatment. Seek care for bloody diarrhoea, severe dehydration signs, or symptoms lasting beyond three days.

Cholera causes profuse watery diarrhoea and life-threatening dehydration within hours. It spreads through contaminated water and food. Safe water, ORT, and prompt medical treatment are essential.

Schistosomiasis (Bilharzia) is caused by parasitic worms entering through skin during contact with contaminated freshwater. Endemic in 42 African countries. Avoid slow-moving freshwater in endemic areas and participate in mass praziquantel treatment programmes.

Typhoid from Salmonella typhi in contaminated water causes prolonged fever and can progress to life-threatening intestinal perforation without antibiotic treatment.

Household Water Treatment Methods

Boiling

Boiling for at least one minute kills all biological pathogens. The most reliable universal method. Store boiled water in a clean, covered container. The main limitation is fuel cost.

Chlorination

Products like WaterGuard, Aquatabs, and SafeWater are formulated for household use. Follow package instructions carefully, allow 30 minutes contact time, and store treated water safely.

Solar Disinfection (SODIS)

Fill clear PET bottles with water and leave in direct sunlight for at least 6 hours (2 days under cloudy conditions). Free and practical in Africa’s sunny climate. Pre-filter turbid water through clean cloth first.

Ceramic and Biosand Filters

Affordable, locally manufacturable systems that effectively remove biological contamination. Widely distributed by NGOs across the continent.

Daily Hydration Goals

  • At least 2–3 litres of total fluid daily in warm African climates
  • Increase during physical activity, illness, pregnancy, breastfeeding, and extreme heat
  • Pale yellow urine indicates good hydration; dark yellow or amber indicates dehydration
  • Children, elderly, and people with kidney conditions are especially vulnerable to dehydration
